A culture of cowardice 

We always tend to avoid responsibility by dividing work among as many employees as we can in order to hide within the queue and the small increments of work. 

However, that creates a culture of cowards and helps enforce fear among the employees; which negatively affects the whole entity. 

It’s a merry-go-round and the only way to switch fields is by creating a culture of bravery.
